Evaluation

PSYCHOLOGICAL EVALUATION REFERRALS

Primary care physicians and therapists who would like to refer their patients to Harmony Mental Health for a psychological evaluation are encouraged to review our External Referral Policy and complete the External Referral Form available below. Please call our office staff at 763-400-8000 with questions about the referral process. It is recommended that individuals verify their insurance coverage for assessment prior to scheduling intake. We do have a waiting list for assessment.

Please call our office staff at 763-400-8000 with questions about the referral process. It is recommended that individuals verify their insurance coverage. We have a waiting list for DBT.
